I use to use Ice Packs to lesson the pain and swelling. Worked well.

Anyone got tips or suggestions to fully denoise Iray renders?

Lighting will fix most of the Iray Noise.
too little light = More noise.
The other thing is post work.
Fill lights and ghost lights help you.

I've heard of a despeckle script that's postscripted to the render, but have yet to find it.
If I do I'll let you know.

The builtin denoise script is rubbish, dont bother.
As others have said lighting is one thing to look at, also the longer it renders the better it will get.
Here are the settings I use to good end:
Pixel Filter Radius: 1.10
Environment Lighting Resolution: 2048
Environment Lighting Blur: on
Minimum Samples: Unlocked and set to 3,000
Maximum Samples: Unlocked and set to 50,000
Maximum time: 75,000
Render Quality: 4.0
And on the advanced tab I set the Med threshold to 2048 and the max to 4096
